What to bring: 

▪ Last year’s Federal and State tax returns and tax paperwork, 

including the Intake/Interview Sheet 

▪ Proof of Identification (Photo ID). For you and your spouse 

▪ Social Security Cards for all taxpayers and dependents on tax return.  

▪ Social Security Benefit Statements (Form SSA-1099) if applicable 

▪ IRS Issued PIN if applicable. 

▪ To minimize your wait time please print the Intake/interview Sheet  

▪  Tax paperwork including items like below if applicable: 

▪ Health Insurance Market Place Statement (Form 1095-A). 

▪ Wage and earnings statements (Such as Form W-2, W-2G.  

1099-R, 1099-Misc, 1099-NEC, 1099-K) 

▪ Bring FINAL Pay Stub if you received overtime during the year. 

▪ Interest and dividend statements (Form 1099-INT, 1099-DIV). 

▪ Documentation of any cash income. 

▪ Charitable Contributions made by cash or check  

▪ Other Itemized Deductions (if exceeding the Standard Deductions) 

▪ Tuition Statement (Form 1098-T). 

▪ Business Expenses and vehicle business mileage if you are self

employed. 

▪ Total paid to a daycare provider, including provider’s Tax 

Identification Number or Social Security Number 

▪ Record of estimated payments to Federal or State for TY2025 

taxes 

▪ Proof of bank routing and account numbers (old or voided check) 

▪ Any Letters from IRS (such as Identity Theft PIN) if applicable. 


UNFORTUNATELY, WE CANNOT PREPARE: 

▪  State returns other than NC 

▪ Schedule C (Business Profit and Loss) if a loss, or if expenses > $50,000, 

or if you have employees. Must take std mileage rate for vehicle. 

▪ Schedule D (Capital Gains and Losses) for other than stocks, mutual 

funds, EFTs, or a personal residence. 

▪ Schedule E (Rental Property, incl VRBO, Airbnb) 

▪ Schedule F (Farm Income) 

▪ Other rental income or business income 

▪ 2106 (Employee Business Expenses) 

▪ 3903 (Moving Expenses) 

▪ 8615 (Minor’s Investment Income) 

▪ Schedule K-1 if it involves depreciation, deductible expenses, business 

income (see 4012 pg D-60) 

▪ Cancellation of Mortgage Debt (1099A and/or 1099C and Form 982) if not 

related to your principal residence 

▪ Disaster Deductions or Relief 

▪ Taxable transactions using virtual currency 

▪ Certain credits (ex. EV’s, Solar, Wind, Geothermal) 

▪ Certain Active Military (check for restrictions)
